ACCESS TO HEALTH RECORDS
To obtain a summary copy of your health records, submit the following information via the contact section of this website
- Name of Person Making Request and Relationship (if not Client)
- Client’s full legal name (Also name given when seen as client if used unmarried or a different name)
- Month(s) and year(s) seen as a client
- Client Date of birth
- Mailing address (Include your address when you were a client if different)
- E-mail address
- Phone and indicate if you can receive text messages
- Reason for request (e.g., coordination of care; changing providers; legal reason)
- Where you wish records to be sent. These are typically sent electronically in encrypted format but can be mailed.
GRIEVANCES AND COMPLAINTS
Gary Kindley is a Licensed Professional Counselor in accordance with the licensing laws for counselors in the State of Texas. Complaints or grievances should be directly addressed with him.
Unresolved complaints regarding a violation of ethics or professional conduct should be filed with the Texas Behavioral Health Executive Council
Complaints must be submitted using the Council’s complaint form (available at the link below).
